Make Sure Your Dependents Meet Pfizer’s Eligibility Requirements
When you enroll or make changes to your enrollment, you will be required to confirm that your dependents meet Pfizer’s eligibility requirements. Pfizer regularly verifies dependents covered under the Pfizer Retiree Medical Plan, and you may be asked to provide proof of eligibility. It is your responsibility to report any changes in your dependents’ status, such as a divorce. Changes must be reported within 31 days of the event to the Pfizer Benefits Center.
The Health Insurance Portability and Accountability Act (HIPAA) includes a Privacy Rule that became effective on April 14, 2003. It requires that employers and health plans protect and secure patients' identifiable health information by controlling who has access to health information and what happens to it.
You will need to complete a HIPPA authorization form if you want to designate someone to have access to your health records. You can download a HIPAA authorization form online in the Reference Library at www.netbenefits.com or request a form by calling the Pfizer Benefits Center at 1-866-476-8723 (or 1-877-208-0950).
